How a Community Link Worker can help you

At Glebe Medical Practice we have a Community Link Worker called Lisa Turner.  A link worker provides non clinical support to patients 18 years and over who are experiencing a negative impact to their mental or physical health due to varying social reasons.  Recognising many issues cannot be resolved by medication, Lisa's role is to support patients to discover and access other resources and services that can help them within their communities.

Many things can affect your wellbeing including:

  • Money worries
  • Family life
  • Employment
  • Education
  • Isolation/loneliness
  • Bereavement
  • Mental health
  • Addiction
  • Weight management
  • Caring for someone

There are a wide range of services and organisations Lisa can refer and signpost to such as:

  • Social work e.g. occupational therapy referrals and care assessment referrals for adults and families
  • Advocacy services e.g. Citizen's Advice Bureau, Money Matters and Voiceability Scotland
  • Mental Health supports e.g. Beacons, Esteem, LAMH
  • Encouragement back to employment e.g. Employability Scotland and Remploy
  • Education e.g. help parents who need support with children who have additional support needs
  • Befriender services for people who are lonely and isolated e.g. Healthy Valleys and Covey
  • Carer and bereavement services e.g. The Haven and CRUS

If you think that speaking to Lisa would be helpful for you, please contact the Surgery and ask to be referred to the Community Link Worker.  Please note that the Patient Care Advisor will ask some questions regarding the reason for referral.
